How to Register

You can now register online using the NHS GP Registration Service, please use the link below:

Please also fill in a GMS1 form which can be printed from the link below or picked up from reception.

When Registering Please Provide:

  • Photo ID – Current Passport or Driving Licence
  • Proof of Address or a Utility Bill
  • National Insurance Card / NHS Number

Temporary Residents

To register as a temporary patient please either contact our reception team on 0115 949 6911 or come into the practice.
